"No such user" when trying to enable 2FA

Problem Description:
When trying to enable 2FA on any new accounts, I get error “No such user”, even when creating new users. I have 2FA setup on other accounts, just wont work for any new users, or current users who dont already have it on.

Steps to reproduce:

  1. go to user settings
  2. try to enable 2FA
  3. get error “no such user”

Actions taken to resolve so far:

enabling / disabling 2FA
restarting instances / server computer
